| What materials to provide for entry |

Complete a separate Entry Form for each promotion and for each category it is entered into. (Using only the Official Entry Form which you can download here) |

Complete the Entry
Summary Control Sheet, detailing all the entries you are making. Attach 5 sets of stapled photocopies of the entry form and the original Client Approval Form, do not photocopy the Client Approval Form. One original copy only is required. |

Calculate the total cost of the entries at the bottom of the Control Sheet and include a cheque, made payable to the 'APMC' for the total amount. Do not forget to add the VAT where applicable. The VAT rate in the Republic of Ireland is 23%. Remember, entries not paid for will not be admissible.

Each entry should be packed separately with the following
material:
(a) Original copy of the Entry
Form including completed Client Approval Form.
(b) Maximum of TWO A2 3ml Cappa Boards featuring printed support material. Clearly indicate the category in which the promotion is being entered by attaching the Entry
ID Label on the reverse of the board. All support materials must fit on the two A2 Cappa Boards. You must supply separate boards for each category entered.
(c)ONE CD containing a jpeg image for all of the entries you have submitted. CD should be marked with the agency name.
(d) ONE CD containing the summaries for each campaign for the awards book and an accompanying image for each campaign for this book- again CD should be clearly marked with the agency name.
(e) Competition Rules and Terms and Conditions of the promotion you are submitting MUST be legible for the judges
